The etruscan Orvieto was destroyed by the Romans in one of their early wars. A [new city](http://upload.wikimedia.org/wikipedia/commons/8/88/Orvieto08_top.jpg) was rebuilt on the original spot on the top of the hill surrounded by cliffs after the collapse of the Roman Empire.
During the Roman empire Italy was unified and protected by the legions, there was little need for defensive strongholds. In the middle ages most cities were independent and there was no central authority to enforce peace, therefore population moved on hilltops and built walls.
